UFCW 21 members who are covered by the Sound Health & Wellness Trust, it is time to save money on your health plan. Each year, Sound Health & Wellness Trust participants and covered spouses 18 years of age and older have an opportunity to take their Personal Health Assessment (PHA) to earn Health Reimbursement Arrangement funding (HRA), if eligible. Taking the PHA is one step to help you avoid a higher deductible and to learn more about your health status.

Eligible plan participants can earn funding up to $500 for employee-only coverage and up to $1,000 for family coverage by annually completing a variety of health-related actions such as the PHA. Your HRA helps you to avoid a higher deductible as HRA funds are used to cover the first eligible medical expenses covered by the plan in each calendar year.

The PHA is available now. Take your PHA, select your primary care physician and confirm your contact information between July 1 – Sept. 30, 2016 to earn 2017 HRA funding. UFCW 21 urges you to take advantage of this opportunity and visit the Sound Health & Wellness Trust web site to get started.
